US citizens receive a 90-day tourist stamp upon arrival in Colombia — more than sufficient for most medical procedures and recovery periods. But some treatments — multi-stage dental restorations, extended rehabilitation programs, stem cell treatment series, or cosmetic surgery recovery with revision potential — may require longer stays. Here's how the extension system works.

The Standard 90-Day Stamp

When you enter Colombia with a US passport, immigration stamps you in for up to 90 days. This is not a visa — it's a tourist permit (permiso de turismo). The 90 days begins on your date of entry, and you don't need to specify a departure date. You're free to stay anywhere in Colombia during this period.

The 90-Day Extension Process

Colombia allows a one-time extension of up to 90 additional days, for a maximum total stay of 180 days within a calendar year. The extension is processed through Migración Colombia, either online or at a Migración office.

The online process (recommended) involves creating an account on the Migración Colombia website, submitting your passport information and current entry stamp details, paying the extension fee, and receiving a digital extension stamp in your passport record. The process has been digitized in recent years, making it far simpler than it used to be.

Documentation for Medical Extensions

While the standard extension doesn't formally require proof of medical treatment, having documentation from your Colombian healthcare provider strengthens your application. A letter from your surgeon or clinic stating that your ongoing medical care in Colombia requires an extended stay can be helpful if questions arise. This letter should include your treatment dates, the nature of the ongoing care, and the expected completion date.

Additionally, you'll need your passport with at least six months of remaining validity, proof of sufficient funds for your extended stay (bank statement or credit card), health insurance documentation (travel insurance or Colombian coverage), and your accommodation details in Colombia.

Cost and Processing Time

The extension fee is approximately 100,000–130,000 COP (roughly $25–$35 USD at typical 2026 exchange rates). The online process typically takes 3–5 business days for approval. Apply at least two weeks before your initial 90-day stamp expires to avoid any timing complications.

If you apply in person at a Migración Colombia office, expect wait times of 1–3 hours. Offices in Medellín (El Poblado location) and Bogotá are the most experienced with foreigner extensions. Bring all documentation in both digital and printed formats.

What Happens at Day 180

The 180-day total (90 initial + 90 extension) is the maximum stay allowed under tourist status within a calendar year. After 180 days, you must leave Colombia. If your medical treatment genuinely requires more time, you have two options: exit Colombia briefly (even a weekend trip to Panama or Ecuador resets your calendar for some purposes, though Migración tracks cumulative stays) or apply for a visa — specifically an M-visa for medical treatment, which is available for patients receiving ongoing care in Colombia.

The M-visa medical category is designed for exactly this situation but involves a more formal application process including medical documentation, financial requirements, and processing at a Colombian consulate or Migración office.

Practical Tips for Extended Recovery Stays

If you know before departure that your treatment will exceed 90 days, plan your extension timeline in advance. Mark calendar reminders for the extension application deadline (apply by day 70 at the latest to ensure processing before expiration). Maintain copies of all medical documentation throughout your stay. Ensure your health insurance covers the extended period — many travel insurance policies cap at 30–90 days and require supplemental coverage for longer trips.

For extended stays, consider transitioning from tourist accommodation (Airbnb, hotel) to a monthly rental. Monthly apartment rates in Medellín's El Poblado or Laureles run $500–$1,200 for a furnished apartment — dramatically cheaper than nightly Airbnb rates and more comfortable for extended recovery.
